So, hypothetically speaking....if you were a middle aged guy with a bum shoulder, and your CJ needed a new clutch, would you
A-pull the front clip and engine, or
    B-pull the trans and tcase

The Jeep in question is a 1970 with stock V6, Saginaw steering, and a T90/twinstick D18/WarnOD.

I'd take it to a transmission place that has the lift, tools, experience, etc., and let them do it. Chalk it up to the cost of getting old. Just my opinion if it was me.

I'm in my mid 60's so don't let middle age scare you. I think I would pull the trans unless the engine needs some work. a lift would be nice but could do it with jack stands and would definitely use a trans jack to raise and lower it.
However it is not a real fun job and if you have the extra cash having it done would be a lot easier.

I may be strange but I just hate it whenever somebody touches a nut or bolt on my rig.
I have a need to know exactly what I'm driving.
    Drop transmission and transfer case.
Try out a makeshift pillow for your shoulder but do not go to sleep under there. Ha Ha Ha !

It all depends on how bad your shoulder really is. Transmission and t-case together if shoulder only bothers you a bit. If worse pay to have it done.

    This.
I do not want anyone working on my jeep, I don't know any shops I'd trust to do it.

    A '70 with a factory V6 should be a T-14, not a T-90. The 1970 V6 factory bell won't fit a T-90.

That said, these components are not terrifically heavy... I did a T-14 R&R in my driveway using just my factory scissors jack. I hefted down the Dana 20 and T-14 aseparately. However, this is when I was much younger. Recently I pulled the T-18 and NP208 from my J20 using the Harbor Freight transmission jack 800 lb. Low Lift Transmission Jack - mine is red, the current model is silver. My only complaint is that the jack is a little tall when fully lowered, so you need to have the car up pretty high to clear the transmission and jack. Should not be a problem with a CJ, but I'd make a measurement before starting to see how high the frame rails need to be.

Another vote for doing it yourself. I had neck/spinal surgery to save losing the function of my left arm due to nerve damage.

Take out the floorboards. Get your friends to come over. Loosen and remove driveline. May have to raise the jeep for clearance.
Rent a low transmission jack. Putting it back in you will need a buddy for sure. One guy to run the jack, another guy to wiggle the transmission around. You can do it with one buddy or two. Mine was a T90. 4 speeds need more buddies.

Would an engine hoist work if you take out the trans cover part of the floor? If you have one. Or maybe an ecj5 work party with friends or local ecj5.com members.

Having done both a transmission pull to replace clutch (along with some other work) and then an engine pull, I would go with transmission removal and leave engine in place. Just my opinion...Its pretty easy to slide the the transmission/D18/OD out on a rolling transmission jack and slide it back in. Hardest part is spending all the time on the floor crawling around - if that sounds really bad on your shoulder then maybe a lift is needed or go the other route. I just know I don't care to pull an engine anytime soon again.

What about using an ATV/Motorcycle jack if you have one?

I had neck/spinal surgery 10 years ago...It hasn't stopped me yet. If I wouldn't have had the surgery, I would have lost the use of my left arm due to nerve damage.

Yes leave the bell attached at the transmission.
I think a come- along that's suspended from a roll bar works about the best..
A sling makes it easy to balance the transmission , transfer case and bell housing assembly.
